  • Copy of the Business Plan
  • Copy of Proposed Lease (if applicable)
  • Copy of Purchase Agreement/ Sales Contract (including legal description) on real estate to be acquired (if applicable)
  • Appraisal on property to be purchased or offered as collateral (or Tax Assessor's Valuation, if appraisal unavailable)
  • List of Equipment to be purchased, including cost
  • Most current 3 years personal tax returns for all owners
  • Current Personal Financial statement for all owners (form included in application)
  • Personal Profile/Resume (form included in application)
  • Most current 2 years tax returns on any affiliate companies.
  • Current Financial Statement (no older than 60 days) on any affiliate companies

If you are applying for a loan for an “existing business”, please complete the application below and attach the following information.

  • Most current 3 years tax returns on the business
  • Business Interim Financial Statement, current within 60 days of application
  • Schedule of Debt, reconciled to the most current balance sheet (form included in application)
  • Copy of Notes/Obligations to be refinanced (if applicable).
  • Copy of Lease (if applicable)
  • Copy of Purchase Agreement/ Sales
  • Contract (including legal description) on real estate to be acquired (if applicable)
  • Appraisal on property to be purchased or offered as collateral (or Tax Assessor's Valuation, if appraisal unavailable)
  • List of Equipment to be purchased, including cost
  • Most current 3 years personal tax returns for all owners
  • Current Personal Financial statement for all owners (form included in application)
